Insurance

Although we accept most major insurances, we advise you to check with your insurance company to ensure that the physician and services are covered. 

Please bring your updated insurance card and identification, such as a driver’s license with you when you come to the office. 

Your office visit will be filed with your insurance company as a courtesy to our patients. You are responsible for the balance that is due for the services rendered.

Unless you have passport insurance, the co-insurance amount will be collected upfront by our front desk receptionist before service is rendered.
