Office Policies

Appointments:

Appointments for sick patients are given priority and can be made by calling the office at (502) 456-4100. Well-check appointments and follow-ups can be made by calling the office or using the Patient Portal. For well-child checks, please limit the number to only two siblings at a time.

Missed Appointment:

A missed appointment benefits no one. We require 24 hours’ notice for cancellation. Please call the office if you need more time to make your appointment. A fee of $55.00 for established patients and $70.00 for new patients may be applied unless there is communication with the office.

Medicine Refills:

All medicine refills are done electronically. Please get in touch with your pharmacy to request your refill; the pharmacy will contact our office for approval. Most requests are answered on the same day, but please allow a 24-hour turnaround during the week. Any request after noon on Fridays will not be answered until the following Monday. For the best care, please request routine refills two to three days before running out of your medication.

Phone Calls:

All phone calls during office hours will be answered by our staff. We are here to help you with all your medical needs. Please call for appointments and medical questions during normal office hours. For urgent calls after hours, please call the main office number, and you will be directed to our answering service.  

If you are having a medical emergency, please call 911 and/or go to the nearest emergency room.

Medical Records:

A signed medical release is necessary before we copy and mail your records to another party. The first copy will be free, but all subsequent copies are $1.00 per page. Second copies will be sent once the records are paid for. Please allow up to 30 days for your request to be processed.
